ABSTRACT

Objetivo: Determinar la frecuencia de calcificación del complejo estilohioideo en radio-grafías panorámicas digitales de un centro radiológico. Métodos. El tipo de estudio fue cualitativo, diseño descriptivo, transversal y retrospectivo; se estudiaron 400 radiografías panorámicas digitales entre edades de 25 a 70 años. Se realizó una evaluación visual de las radiografías panorámicas digitales donde se observó el tipo y patrón de calcificación del ligamento estilohioideo. Resultados. El 56,8% presentó calcificación del complejo estilohioideo en radiografías panorámicas digitales; según sexo en el grupo femenino se presentó en un 64,6%; respecto al grupo etario el 65.8% presentó calcificación en adultos de 30 a 59 años; según el lado afectado el 71,4% fue bilateral, según la apariencia radiográfica el tipo I presentó una mayor frecuencia con un 50,1% en el lado derecho y el 62,5% en el lado izquierdo; finalmente el patrón de calcificación más frecuente fue el completamente calcificado con un 23,8% en el lado derecho y un 48,5% en el lado izquierdo. Conclusión. La frecuencia de calcificación del complejo estilohioideo en ra-diografías panorámicas digitales en la muestra estudiada fue alta por lo que es importante el uso de la radiografía panorámica como medio de diagnóstico para poder detectar este tipo de hallazgos que junto a una evaluación clínica nos permitan hacer un correcto diagnóstico en fin de un tratamiento adecuado.


Objective: To determine the frequency of the stylohyoid complex calcification in digital panoramic radiographs of a radiological center. Methods.The type of study was quali-tative, descriptive, cross-sectional and retrospective design; 400 digital panoramic radio-graphs between the ages of 25 and 70 were analyzed. A visual evaluation of the digital panoramic radiographs was performed to assess the type and pattern of the stylohyoid ligament calcification. Results. The 56.8% presented calcification of the stylohyoid com-plex in digital panoramic radiographs; according to sex in the female group it was present in 64.6%; Regarding the age group, 65.8% presented calcification in adults aged 30-59 years-old; According to the affected side, 71.4% were bilateral, according to the radio-graphic appearance, type I presented a higher frequency with 50.1% on the right side and 62.5% on the left side. Finally, the most frequent calcification was the completely calcified pattern, with 23.8% on the right side and 48.5% on the left side. Conclusion.The frequency of calcification of the stylohyoid complex in digital panoramic radio-graphs in the sample studied was high, so it is important to use panoramic radiography as a means of diagnosis to be able to detect this type of findings that, together with a clinical evaluation, allow us to make a correct diagnosis and perform an adequate treatment.

ABSTRACT

La coloración rosa de los dientes puede originarse por diferentes factores. En el ámbito forense se ha descrito al fenómeno denominado post mortem pink teeth como un signo asociado a muertes violentas de etiología diversa. En la práctica clínica también es posible observar pacientes con dientes rosados, fre-cuentemente ocasionados por traumatismos o iatro-genia proveniente de ortodoncia, cuyo mecanismo de producción obedece a distintas etiopatogenias, destacándose las reabsorciones dentinarias inter-nas, cemento-dentinarias externas y calcificaciones dentinarias. El presente artículo expone el caso de un individuo adulto con antecedente de trauma óseo-dentario por accidente vial que, luego de un prolon-gado tiempo, asiste al Servicio de Urgencias Odon-tológicas y Orientación de Pacientes de la Facultad de Odontología de la Universidad de Buenos Aires, en donde se le detecta, a modo de hallazgo exploratorio, una ostensible coloración rosada en el canino infe-rior derecho. La situación motivó un pormenorizado abordaje clínico y radiográfico, indagando respecto a los probables factores que intervinieron en su ge-neración y desarrollo (AU)


The pink coloration of the teeth can be caused by dif-ferent factors. In the forensic field, the phenomenon called post mortem pink teeth has been described as a sign associated with violent deaths of various etiology. In clinical practice, it is also possible to ob-serve patients with pink teeth, frequently caused by trauma or iatrogenesis from orthodontics, whose production mechanism is due to different etiopatho-genesis, highlighting internal dentin resorption, ex-ternal cemento-dentinal resorption and dentin calci-fications. This article presents the case of an adult individual with a history of bone-dental trauma due to a road accident who, after a long time, attends the Dental Emergency and Patient Guidance Service of the Faculty of Dentistry of the University of Bue-nos Aires, where an ostensible pink coloration was detected in the lower right canine as an exploratory finding. The situation motivated a detailed clinical and radiographic approach, inquiring about the probable factors that intervened in its generation and development (AU)

ABSTRACT

La pancreatitis crónica (PC) es una enfermedad de incidencia creciente en la población pediátrica, aunque desconocida, y conlleva una importante comorbilidad. La clínica inicial suele ser la propia de un episodio de pancreatitis aguda pero la importancia recae en el desarrollo, al cabo de meses o años, de insuficiencia pancreática. Para su diagnóstico será necesario tener un alto índice de sospecha clínica y realizar pruebas de imagen que demuestren cambios propios de la PC. El manejo de esta entidad deberá ser multidisciplinar y consistirá en controlar de forma adecuada el dolor e iniciar tratamiento sustitutivo cuando se constate la presencia de insuficiencia pancreática exocrina y endocrina. El caso que se reporta a continuación es un buen ejemplo de un caso de PC con presencia de insuficiencia exocrina y endocrina. Se documenta el proceso diagnóstico de la PC, en cuanto a pruebas de imagen y pruebas que demuestren la presencia de insuficiencia pancreática. Se exponen también las diferentes pruebas que se realizan para llegar a la etiología del cuadro. Se intenta reflejar el manejo que se debe llevar a cabo en estos pacientes y el seguimiento estricto que requieren.


Chronic pancreatitis (CP) is a disease with increasing incidence, though unknown, in the last years in the pediatric population which entails important comorbidities. The initial symptomatology is similar to the clinical scenario of acute pancreatitis, but its importance relies on the development, in months to years, of pancreatic insufficiency. For its diagnosis a high clinical suspicion and typical image findings are necessary. The management of this entity is held by a multidisciplinary team and the main goal is to assure an optimal pain control and start substitutive treatment once the pancreatic insufficiency is diagnosed. The clinical case that follows is a good example of CP with stablished pancreatic exocrine and endocrine insufficiency. It documents the diagnostic process of CP, regarding the imaging tools used and other laboratory test that manifested the pancreatic insufficiency. We expose the different etiologic tests that were performed in order to get to the etiologic diagnosis. It intends to reflect on the management needed for these patients and the strict follow-up that must be done.

This study was conducted to evaluate the sensitivity of Magnetic Resonance (MR) Susceptibility- Weighted Imaging (SWI) and also to compare the sensitivity of Magnetic Resonance (MR) susceptibility-Weighted Imaging (SWI) and Computed Tomography (CT), such that SWI can replace the use of CT as a standard protocol in the detection and differentiation between calcification and hemorrhage. A series of 70 patients included in this study with clinical suspicion or known history of intracranial hemorrhage/ calcifications for follow up, were scanned using both Philips Multiva 1.5T (MRI/SWI) and Philips Ingenuity 128 slice (CT). Results showed that SWI had 100% relative sensitivity, as it was able to detect both hemorrhage and calcifications in all the cases identified in CT. In addition, SWI detected hemorrhage in 2 additional cases that were nonspecific in CT. Furthermore, SWI detected signal from micro bleeds, which was missed on CT in three cases, and also detected calcification in two cases, which were not positively identified in CT. In conclusion, Susceptibility- Weighted Imaging (SWI) is highly sensitive for the detection and differentiation of hemorrhage and calcifications using Phase reconstructed images compared to conventional MRI methods and Computed Tomography

ABSTRACT

Abstract Cardiovascular disorders represent the leading cause of death in dialysis patients. Alterations of bone and mineral metabolism (BMM) and vascular calcifications play a fundamental role in it. The objective of this study was to evaluate the predictive role on cardiovascular mortality of the measurement of biomarkers of BMM and vascular calcifications. A prospective cohort study was performed. All prevalent patients on chronic dialysis in September 2009 at our institution, who completed the total of the complementary stud ies, were studied. BMM biomarkers were measured (FGF 23, fetuin A, PTH, calcium and phosphorus) and the vascular calcifications were evaluated using the Kauppila and Adragao scores. Follow-up was carried out until 1/1/2019, death or transplant. Of the 30 patients included, 7 (23.3%) died due to cardiovascular causes. The follow-up time was 44.1 ± 30.4 (range = 1.4-112) months. The Adragao score was the only predictive variable of long-term cardiovascular mortality (area under the curve = 0.82; 95% CI 0.64-0.94; p < 0.001). The best cut-off point was 5 (sensitivity = 85.7%; specificity = 78.3%). It was also an independent risk factor for cardiovascular mortality adjusted for age, diabetes mellitus, coronary heart disease, aortic calcifications, time spent on dialysis and follow-up time (adjusted OR = 1.77; 95% CI = 1.06-2.96; p = 0.028). The vascular calcifications quantified from the Adragao score were the only independent predictor of long-term cardiovascular mortality. This score represents a simple, useful and superior tool to the biomarkers of BMM.


Resumen Los trastornos cardiovasculares representan la primera causa de muerte en los pacientes en diálisis. Las alteraciones del metabolismo óseo y mineral (MOM) y las calcificaciones vasculares juegan un papel fundamental en la misma. El objetivo de este estudio fue evaluar el rol predictor sobre la mortalidad car diovascular de la medición de los biomarcadores del MOM y las calcificaciones vasculares. Se realizó un estudio de cohorte prospectivo. Se estudiaron todos los pacientes prevalentes en diálisis crónica en septiembre del 2009 en nuestra institución que completaron el total de los estudios complementarios. Se midieron biomarcadores del MOM (FGF 23, fetuína A, PTH, calcio y fósforo) y se evaluaron las calcificaciones vasculares mediante los scores de Kauppila y de Adragao. Se realizó un seguimiento hasta el 1/1/2019, la muerte o el trasplante. De los 30 pacientes incluidos, 7 (23.3%) fallecieron por causa cardiovascular. El tiempo de seguimiento fue de 44.1 ± 30.4 (rango = 1.4-112) meses. El score de Adragao fue la única variable predictiva de muerte cardiovascular a largo plazo (área bajo la curva = 0.82; IC95% = 0.64-0.94; p<0.001). El mejor punto de corte fue de 5 (sensibili dad = 85.7%; especificidad = 78.3%). Además, fue un factor de riesgo independiente de muerte cardiovascular ajustado por edad, diabetes mellitus, enfermedad coronaria, calcificaciones aorticas, tiempo de permanencia en diálisis y tiempo de seguimiento (OR ajustado = 1.77; IC95% = 1.06-2.96; p = 0.028). Las calcificaciones vasculares cuantificadas a partir del score de Adragao fueron el único predictor independiente de mortalidad cardiovascular a largo plazo. Este score representa una herramienta simple, útil y superior a los biomarcadores del MOM.

RESUMEN: La radiografía panorámica es probablemente el examen de rutina más solicitado por los dentistas en el mundo. Entre sus ventajas están su bajo costo, rapidez y mayor seguridad debido a que los formatos digitales han permitido disminuir las dosis y tiempos de exposición a la radiación. Su potencial como herramienta diagnóstica radica en la posibilidad que tiene el clínico de observar el territorio maxilofacial casi en su totalidad, pudiendo realizar un estudio anatómico y morfológico acabado de diferentes regiones. En esta revisión se quiere ofrecer una mirada general de la amplia gama de usos que se le pueden dar a la radiografía panorámica, tanto en la clínica como en el ámbito de la investigación, destacando su potencial como herramienta de diagnóstico y su importancia para algunas áreas de especialidad en odontología.


SUMMARY: The panoramic radiograph is in all likelihood, the routine exam most required by dentists in the clinic worldwide. Among the advantages of using this type of radiograph, is that it is inexpensive, quick and also safer in digital format since it reduces the radiation dose and time of exposure. Its potential as a diagnostic tool is based on the possibility to explore almost all of the maxillofacial area, allowing a proper anatomic and morphological study of different areas at the same time. In this review, we aimed to provide an overview of the multiple uses of panoramic radiographs, highlighting its potential as a diagnostic tool and also its importance in some specialized areas of dentistry.

ABSTRACT

Objetivos: Estimar la prevalencia de nódulos pulpares y agujas cálcicas en pacientes atendidos en la Cátedra de Endodoncia, Facultad de Odontología, Universidad de Buenos Aires (CEFOUBA). Se analizaron radiografías de 1866 piezas dentarias, con ápices maduros, de pacientes de ambos géneros, mayores de 15 años, tratados en CEFOUBA, en el período comprendido entre junio-noviembre de 2018. Género, edad, grupo dentario, condición del diente, presencia de patología sistémica, y ubicación anatómica, fueron las variables registradas. Se calcularon intervalos de confianza 95%, método score de Wilson. Se utilizó test z para diferencias de proporciones. El porcentaje de piezas con calcificaciones superó significativamente a piezas sin calcificaciones (z= 6,076; P<0,05). Porcentaje de piezas en mujeres fue significativamente mayor al de hombres (z=24,31; P<0,05). Porcentaje de piezas de las cuartas y quitas décadas de la vida (61%) superó significativamente a las demás décadas (39%) (z=9,88; P<0,05). El porcentaje de piezas con caries fue significativamente mayor al resto de las piezas con otras condiciones (41%) (z= 8,074; P<0,05). El porcentaje de piezas en pacientes sanos (77%) fue significativamente mayor al resto de las condiciones (23%) (z=24,310; P<0,05). El porcentaje de calcificaciones en cámara (78%) fue significativamente mayor a las del conducto (22%) (z=25,21; P<0,05). En conclusión, la presencia de este tipo de calcificaciones es frecuente en molares de pacientes femeninos y edad adulta. Es importante detectarlos radiográficamente porque complican la realización del tratamiento endodóntico, especialmente si están adheridos y obliteran la entrada de los conductos (AU)

ABSTRACT

Objective:To explore the diagnostic value of breast MRI in breast imaging reporting and data system (BI-RADS) 4 mammography-detected ultrasound-negative (MG+/US-) pure calcification lesions.Methods:The data of 51 patients (52 calcification lesions) who were diagnosed with BI-RADS 4 pure calcification by mammography, had no significant abnormality on ultrasonography, and finally obtained pathological results at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from December 2013 to December 2019 were retrospectively analyzed. Of the 52 calcification lesions, 40 were benign and 12 were malignant. The mammographic findings (breast composition, calcification morphology, and distribution) and MR findings (enhancement pattern), of benign and malignant lesions were analyzed and compared with the histopathological results. The diagnostic performance of MRI was calculated.Results:There was a significant difference between benign and malignant calcification lesions in calcification morphology on mammography and enhancement pattern on MRI ( P<0.05). On breast MRI, malignant calcifications lesions showed mass or non-mass enhancement (12/12), while benign calcified lesions mostly showed no enhancement or punctate enhancement (27/40). The sensitivity, specificity, positive predictive value, and negative predictive value of breast MRI in the diagnosis of BI-RADS 4 MG+/US- pure calcification lesions were 66.7% (8/12), 75.0% (30/40), 44.4% (8/18) and 88.2% (30/34), respectively; The sensitivity, specificity, positive predictive value and negative predictive value for BI-RADS 4A calcifications were 66.7% (2/3), 74.2% (23/31), 20.0% (2/10) and 95.8% (23/24), respectively. The sensitivity, specificity, positive predictive value and negative predictive value for BI-RADS 4B calcifications were 66.7% (4/6), 85.7%(6/7),80.0% (4/5) and 75.0% (6/8), respectively. Conclusions:MRI has a high differential diagnostic value for mammographic BI-RADS 4 pure calcifications with ultrasound-negative, especially for BI-RADS 4A calcifications. Follow-up instead of biopsy is recommended for lesions with no enhancement or punctate enhancement on MRI.

ABSTRACT

Resumen Varón de 33 años con antecedentes de convulsiones febriles y discapacidad intelectual moderada grave, inició a los 2 años convulsiones tanto focales como generalizadas atónicas diarias, que ocasionaron traumas faciales. Debido a la farmacorresistencia se implantó un estimulador del nervio vago, con respuesta parcial al mismo. Durante su seguimiento, se hizo diagnóstico de enfermedad celíaca. Al realizar una tomografía de encéfalo se evidenciaron calcificaciones piriformes occipitales bilaterales, estableciéndose el diagnostico de enfermedad celiaca, epilepsia y calcificaciones cerebrales. Se le indicó dieta libre de gluten y continuar el tratamiento farmacológico, logrando de esta manera una reducción de las crisis.


Abstract A 33-year-old man with a history of febrile seizures and moderate-severe intellectual disability began, at 2 years, both focal and generalized daily atonic seizures, which caused facial trauma. Due to drug resistance, a vagus nerve stimulator was implanted, with partial response to it. During his follow-up, he was diagnosed with celiac disease. When performing a brain tomography, bilateral occipital pyriform calcifications were evidenced, establishing the diagnosis of celiac disease, epilepsy and cerebral calcifications. A gluten-free diet was indicated and pharmacological treatment continued, thus achieving a reduction in seizures.

ABSTRACT

INTRODUCCIÓN: La filariasis con afectación mamaria es una enfermedad endémica de áreas tropicales y subtropicales de África, Asia, el Pacífico y América, que afecta a unos 120 millones de personas. Aunque es una patología rara en España, dado el aumento de pacientes procedentes de dichos países, debemos conocerla para saber diagnosticarla y tratarla adecuadamente. OBJETIVO DE REPORTAR EL CASO: Dar a conocer la filariasis con afectación mamaria, sus manifestaciones clínicas y radiológicas principales, a través de un caso clínico de nuestras consultas de ginecología. DESCRIPCIÓN DEL CASO: Paciente de 43 años, procedente de Guinea Ecuatorial que acude a la consulta de Ginecología por mastalgia bilateral y aumento del volumen de las mamas de semanas de evolución. La exploración es anodina por lo que se solicita mamografía bilateral en la que describen calcificaciones compatibles con filariasis. A pesar de que el resto de pruebas fueron negativas, dada la alta sospecha clínica y radiológica se diagnosticó de filariasis mamaria. CONCLUSIONES: A pesar de la actual campaña mundial para eliminar la filariasis, el aumento de migración global incrementa la probabilidad de padecer casos importados de filariasis mamaria. Por tanto, el conocimiento de las diferentes parasitosis es imprescindible para realizar un buen diagnóstico diferencial con otras entidades clínicamente similares, e instaurar el tratamiento más adecuado.


INTRODUCTION: Breast filariasis is an endemic disease from tropical and subtropical areas of Africa, Asia, the Pacific and America affecting about 120 million people. Although it is a rare pathology in Spain, given the increase in patients from the referred countries, we must be aware of it in order to know how to diagnose and treat it properly. OBJECTIVE OF REPORTING THE CASE: To raise awareness of filariasis with breast involvement and its main clinical and radiological manifestations, through a clinical case of our gynecology consultations. CASE REPORT: 43-year-old patient from Equatorial Guinea who comes to the Gynecology consultation for bilateral mastalgia and breast enlargement of weeks of evolution. The examination is anodyne, so bilateral mammography is requested in wich calcifications compatible with filariasis are described. Despite the rest of the tests are negative, given the high clinical and radiological suspicion, the diagnosis of breast filariasis is made. CONCLUSIONS: Despite the current worldwide campaign to eliminate filariasis, the increase in global migration enhances the probability of suffering from imported cases of breast filariasis Therefore, the knowledge of the different parasitoses is essential to make a correct differential diagnosis with other clinically similar entities, and to establish the most appropriate treatment.

ABSTRACT

Abstract Sclerochoroidal calcifications (SC) are a rare and benign ocular condition characterized by yellow-white irregular subretinal lesions usually found in the supero-temporal arcade of the midperipheral fundus in middle-aged elderly men. We present a clinical case of a 79- year-old patient who during a fundus examination presented raised whitish nodules in the supero-temporal arcade in the right eye. After performing optical coherence tomography, ultrasound, ocular computed tomography and laboratory analysis, she was diagnosed with idiopathic sclerochoroidal calcifications The pathogenesis of sclerochoroidal calcifications remains unclear but systemic conditions should be discarded. It is important to distinguish sclerochoroidal calcifications from other conditions such as tumors.


Resumo Calcificações esclerocoroidais (SC) são uma condição ocular rara e benigna caracterizada por lesões sub-retinianas irregulares amarelo-brancas, geralmente encontradas na arcada superotemporal do fundo médio-periférico em homens idosos de meia-idade. Apresentamos um caso clínico de uma paciente de 79 anos que durante exame de fundo apresentou nódulos esbranquiçados elevados na arcada superotemporal do olho direito. Após realizar tomografia de coerência óptica, ultra-sonografia, tomografia computadorizada ocular e análise laboratorial, ela foi diagnosticada com calcificações esclerocoroidais idiopáticas A patogênese das calcificações esclerocoroidais permanece incerta, mas as condições sistêmicas devem ser descartadas. É importante distinguir calcificações esclerocoroidais de outras condições, como tumores.

Abstract Intracranial calcifications, which are common in the daily routine of radiologists, can have a physiological or pathological origin. Determining the cause of intracranial calcifications can represent a challenge. The anatomical location, distribution, dimensions and morphology of such calcifications are important findings, which, in conjunction with the clinical history and age group, can facilitate the differential diagnosis. The aim of this pictorial essay is to demonstrate the different types of intracranial calcifications and their origins. The images evaluated were those stored in picture archiving and communication systems. All of the cases included were studied by computed tomography, magnetic resonance imaging, or both. We identified, classified, and described 64 types of intracranial calcifications.


Resumo Calcificações intracranianas fazem parte da rotina do médico radiologista, as quais podem ter origem fisiológica ou patológica. A diferenciação entre as calcificações pode representar um desafio. Localização anatômica, distribuição, dimensões e morfologia das calcificações são importantes achados que, associados a história clínica e faixa etária, podem ajudar no diagnostico diferencial. O objetivo deste estudo é demonstrar os diferentes tipos de calcificações intracranianas e suas origens. Foram avaliadas as imagens armazenadas no sistema de comunicação e arquivamento de imagens das unidades. Todos os casos inclusos foram estudados por tomografia computadorizada e/ou ressonância magnética. Foram encontrados 64 tipos de calcificações intracranianas, que foram classificadas e descritas.

ABSTRACT

Fahr’s disease is a disorder characterized clinically by a wide spectrum of varied clinical-neurological and psychiatric manifestations occurring secondary to intracranial calcifications with subsequent neuronal cell loss. Though the disease can present in early childhood or adolescence the usual age of manifestation is around 4th-5th decades of life. We report a series of 6 Fahr’s disease cases with respect to different clinical and radiological manifestations. The details of different clinical manifestations with respect to the disease were studied. The frequency of symptoms, the radiological pattern of intracranial calcifications and the association of different parameters were studied.  Progressive cognitive decline and Parkinsonism was detected in all the patients but in none of them it was the chief presenting feature. Seizure was presenting symptoms in 3 patients. Chorea was encountered in 2 patients as the presenting complaint. Mild wide-based cerebellar ataxic gait was found in only one patient but other cerebellar signs were absent. Athetosis, dyskinesia, or dystonia was present in none of our patient. CT scan revealed symmetric basal nuclei and cerebellar calcification in all patients. The disease needs a high index of suspicion and CT brain scanning should always be performed in patients younger than 50 years who present with refractory seizures, Parkinsonism and cognitive decline. However radiological findings did not predict the presentation and outcome.

Introducción. El hipoparatiroidismo es una enfermedad caracterizada por la ausencia o concentraciones inadecuadamente bajas de hormona paratiroidea (PTH), que conduce a hipocalcemia, hiperfosfatemia y excreción fraccional elevada de calcio en la orina. Las calcificaciones del sistema nervioso central son un hallazgo frecuente en estos pacientes. Caso clínico. Mujer de 56 años con antecedente de hipotiroidismo, que ingresó por un cuadro de 6 días de evolución caracterizado por astenia, parestesias periorales y movimientos anormales de manos y pies. Las pruebas de laboratorio demostraron hipocalcemia, hiperfosfatemia y niveles bajos de hormona paratiroidea. Se realizó una tomografía computarizada de cráneo que mostró áreas bilaterales y simétricas de calcificaciones en hemisferios cerebelosos, ganglios basales y corona radiata. No se evidenciaron trastornos en el metabolismo del cobre y hierro. Se estableció el diagnóstico del síndrome de Fahr secundario a hipoparatiroidismo y se inició tratamiento con suplementos de calcio y vitamina D con evolución satisfactoria. Discusión. El síndrome de Fahr es un trastorno neurológico caracterizado por el depósito anormal de calcio en áreas del cerebro que controlan la actividad motora. Se asocia a varias enfermedades, especialmente, hipoparatiroidismo. La suplementación con calcio y vitamina D con el objetivo de normalizar los niveles plasmáticos de estos cationes es el tratamiento convencional. (AU)


Introduction. Hypoparathyroidism is a disease characterized by absence or inappropriately low concentrations of circulating parathyroid hormone, leading to hypocalcaemia, hyperphosphataemia and elevated fractional excretion of calcium in the urine. Central nervous system calcifications are a common finding in these patients. Case report. 56-year-old woman with a history of hypothyroidism who was admitted for a 6-day course of illness characterized by asthenia, perioral paresthesias, and abnormal movements of the hands and feet. Laboratory tests showed hypocalcemia, hyperphosphatemia, and low parathyroid hormone levels. A cranial computed tomography was performed. It showed bilateral and symmetrical areas of calcifications in the cerebellar hemispheres, basal ganglia, and radiata crown. No disorders of copper or iron metabolism were evident. The diagnosis of Fahr syndrome secondary to hypoparathyroidism was established and treatment with calcium and vitamin D supplements was started with satisfactory evolution. Discussion. Fahr's syndrome is a neurological disorder associated with abnormal calcium deposition in areas of the brain that control motor activity. It is associated with various diseases, especially hypoparathyroidism. The conventional treatment is supplementation with calcium and vitamin D, with the aim of normalizing their plasma levels. RESUMEN Introducción: Las células de la pulpa dental tienen la capacidad, ante estímulos locales y sistémicos, de activar a la osteopontina, formando calcificaciones pulpares, bloqueando los conductos radiculares y modificando la anatomía interna; de manera que se intensifican en personas con diabetes mellitus. Objetivo: Determinar si existe mayor presencia de calcificaciones a nivel de cámara pulpar en pacientes con diabetes mellitus versus pacientes sin diabetes mellitus. Métodos: Se realizó un estudio de casos y controles, analítico, transversal y retrospectivo. Se utilizaron los registros clínicos y las radiografías digitales periapicales de pacientes de las clínicas odontológicas de pregrado (Prótesis, Restauradora, Integral, Periodoncia) y de postgrado (Periodoncia e Implantología quirúrgica) de la Universidad Andrés Bello - Chile, entre los años 2017 y 2018. Se seleccionó una muestra de 617 dientes (45 pacientes) luego de ser aplicados los criterios de exclusión e inclusión en la base de datos clínica. La muestra completó dos grupos, grupo de casos (22 pacientes): dientes de pacientes con diabetes mellitus 228 dientes indemnes y el grupo de control (23 pacientes): dientes de pacientes sin diabetes mellitus 389 dientes indemnes. Se realizó una calibración previa entre los investigadores para la identificación y clasificación de calcificaciones pulpares durante el análisis radiográfico de estas. Se realizaron inferencias estadísticas de chi cuadrado para establecer significación de los resultados obtenidos. Resultados: Los dientes de 22 pacientes diabéticos presentaron más calcificaciones camerales que los dientes de 23 pacientes no diabéticos, para 50 por ciento de dientes calcificados en pacientes diabéticos (n=114) y 26 por ciento en pacientes no diabéticos (n=101); este resultado fue estadísticamente significativo (pvalor< 0,001). Conclusiones: Los pacientes con diabetes mellitus demostraron que son más propensos a formar calcificaciones camerales debido al aumento de glucosa presente en la sangre que activa a la osteopontina(AU)


ABSTRACT Introduction: Dental pulp cells have the capacity to respond to local and systemic stimuli by activating osteopontin, forming pulp calcifications, blocking root canals and modifying internal anatomy, in such a way that they intensify in people with diabetes mellitus. Objective: Determine whether there is a greater presence of calcifications in the pulp chamber of patients with diabetes mellitus vs. patients without diabetes mellitus. Methods: An analytical cross-sectional retrospective case-control study was conducted of medical records and digital periapical radiographs of patients attending undergraduate dental clinics (prosthesis, restoration, comprehensive, periodontics) and graduate dental clinics (periodontics and surgical implantology) of Andrés Bello University, Chile, in the period 2017-2018. A sample was selected of 617 teeth (45 patients) from the clinical database upon applying exclusion and inclusion criteria. The sample was divided into two groups: a case group, formed by 228 undamaged teeth from 22 patients with diabetes mellitus, and a control group, formed by 389 undamaged teeth from 23 patients without diabetes mellitus. Prior calibration was performed by the researchers to identify and classify the pulp calcifications during their radiographic analysis. Chi-squared statistical inferences were made to establish the significance of the results obtained. Results: The teeth from the 22 diabetic patients had more chamber calcifications than the teeth from the 23 nondiabetic patients, for 50 percent calcified teeth in diabetic patients (n= 114) and 26 percent in nondiabetic patients (n= 101). This result was statistically significant (p-value< 0.001). Conclusions: Patients with diabetes mellitus were found to be more prone to form chamber calcifications, due to the increase in glucose present in the blood which activates osteopontin(AU)

@#<p><strong>OBJECTIVE: </strong>To determine the association of thyroid malignancy and the following ultrasonographic findings: presence of solid hypoechoic nodule, irregular margins (infiltrative, microlobulated, or speculated), microcalcifications or disrupted rim calcifications with small extrusive hypoechoic soft tissue component, taller than wide shape of the thyroid nodule, and evidence of extrathyroidal extension.</p><p><strong>METHODS:</strong></p><p><strong>Design:           </strong>Cross-Sectional Study</p><p><strong>Setting:           </strong>Tertiary Government Training Hospital</p><p><strong>Participants: </strong>Records of patients admitted to the Otorhinolaryngology- Head and Neck Surgery ward with a diagnosis of nodular non-toxic goiter, multinodular non-toxic goiter, and thyroid malignancy who underwent thyroid surgery between January 2017 and June 2018 were considered for inclusion.</p><p><strong>RESULTS: </strong>A total of 33 patients, 7 males and 26 females, were included in this study. Patients' age ranged from 26 to 69 years with an average of 46 years. Thirteen (39.4%) had malignant while 20 (60.6%) had benign histopathologic results. There was a significant association between presence of solid hypoechoic nodule (Fisher exact, n = 33, p = .0047), irregular margins and microcalcifications with malignant histopathology results (X2 (df = 1, N = 33) = 8.57, p = .003). No significant difference was noted in the proportion of subjects with malignancy according to taller than wide nodules (Fisher exact, n = 33, p = 1.000) or presence of extrathyroidal extension nodules or presence of extrathyroidal extension (Fisher exact, n = 33, p = .276). On multivariate analysis using logistic regression, only microcalcification was found to be a significant predictor of malignancy (OR = 8.96, 95% CI:  1.02 - 87.19, p = .05).</p><p><strong>Conclusion: </strong>There was a significant association between presence of solid hypoechoic nodule, margins and microcalcifications with thyroid carcinoma. Only microcalcification was found to be a significant predictor of thyroid malignancy on ultrasound, although our confidence interval was broad.</p><p><strong>KEYWORDS:</strong> calcifications; thyroid; thyroid carcinoma; thyroid nodule; ultrasonography</p>

Among 12 cases, 4 were revealed as benign breast diseases and 8 were diagnosed as breast cancer.@*Conclusions@#Ultrasound guided mammotome biopsy is found to be an alternative method to stereotactic biopsy in patients with US-detectable micro-calcifications, and re-scan ultrasonography focusing on the specific microcalcification area may be helpful for improving the ultrasound detection rate of micro-calcifications.

Objective To investigate the applicafion value of a simple stereotaxic technology in open biopsy of breast calcification with wire localization guided by X-ray.Methods 36 lesions in 30 cases with calcifications on mammography underwent X-ray guided simple wire stereotaxic localization open biopsy.Results 36 lesions in 30 cases were removed completely after wire localization,among which 24 cases were removed completely by one time,accounting for 80% (24/30)of the total,and 6 cases were removed completely by two times,accounting for 20% (6/30)of the total.No case were removed without calcification or removed completely by more than three times.Conclusion The simple stereotaxic technology in open biopsy of breast calcification with wire localization guided by X-ray is easy to learn and safe to do,which is suitable for promotion at the grassroots hospital.

ABSTRACT

OBJECTIVE: To develop a scoring system stratifying the malignancy risk of mammographic microcalcifications using the 5th edition of the Breast Imaging Reporting and Data System (BI-RADS).MATERIALS AND METHODS: One hundred ninety-four lesions with microcalcifications for which surgical excision was performed were independently reviewed by two radiologists according to the 5th edition of BI-RADS. Each category's positive predictive value (PPV) was calculated and a scoring system was developed using multivariate logistic regression. The scores for benign and malignant lesions or BI-RADS categories were compared using an independent t test or by ANOVA. The area under the receiver operating characteristic curve (AUROC) was assessed to determine the discriminatory ability of the scoring system. Our scoring system was validated using an external dataset.RESULTS: After excision, 69 lesions were malignant (36%). The PPV of BI-RADS descriptors and categories for calcification showed significant differences. Using the developed scoring system, mean scores for benign and malignant lesions or BI-RADS categories were significantly different (p < 0.001). The AUROC of our scoring system was 0.874 (95% confidence interval, 0.840–0.909) and the PPV of each BI-RADS category determined by the scoring system was as follows: category 3 (0%), 4A (6.8%), 4B (19.0%), 4C (68.2%), and 5 (100%). The validation set showed an AUROC of 0.905 and PPVs of 0%, 8.3%, 11.9%, 68.3%, and 94.7% for categories 3, 4A, 4B, 4C, and 5, respectively.CONCLUSION: A scoring system based on BI-RADS morphology and distribution descriptors could be used to stratify the malignancy risk of mammographic microcalcifications.
